Job Summary

Performs all clerical functions for a physician in the hospital or clinic setting.  Assists the physician in diagnostic testing, examination, education and treatment of patients in a Cardiology Inpatient and Outpatient setting.

Essential Functions

  1. Scribing for the Cardiology providers in the medical record.

  2. Collects data accurately and sees that all information is placed in the patient’s electronic medical record.

  3. Uses effective communication skills and interpersonal skills in obtaining information from patient.

  4. Maintains patient confidentiality.

  5. Instructs patients and their family members in home care as outlined by the physician.

  6. Assists with health education and rehabilitation on the patient’s level of understanding.


Key Responsibilities:

  • Chart in real-time as the provider assesses and examines the patient.
  • Document the history, physical exam, assessment and plan, results, procedures, treatment, and physician consultations.
  • Record provider-dictated diagnoses, prescriptions and instructions for patient follow-up.
  • Locate and obtain past medical histories, previous charts, past results and studies.
  • Transcribe ancillary test results and their interpretations.
  • Check on the progress of labs, imaging studies, and other patient data.
  • Retrieve and display current and past EKGs and imaging films (e.g. X-rays) for the provider to review and record provider’s interpretation.
  • Understand basic ICD-10 documentation requirements.
  • Dictation/faxing/phone calls and clerical tasks. Medical Scribe Assistant is asked to prepare referral letters as directed by the physician.
  • Contact information for referring physicians, make phone calls, and other clerical tasks.
  • Medical Scribe Assistant spots mistakes or inconsistencies in medical documentation and checks to correct the information in order to reduce errors.  All addenda must be signed off by a physician. Medical Scribe Assistant ensures that all clinical data, lab or other test results, the interpretation of the results by the physician are recorded accurately in the medical record.  Alerts physician when chart is incomplete. Medical Scribe Assistant must comply with specific standards that apply to the style of medical records and to the legal and ethical requirements for preparing medical documents and for keeping patient information confidential.
  • Proofreads and edits all the physician’s medical documents for accuracy, spelling, punctuation, and grammar.
  • Makes corrections/revisions/addenda to transcribed documents as needed.
  • Transcribes documents in prioritized order established by the department.
  • Able to provide care for patients of all age groups.  Maintains age specific competencies.
  • Complies with all regulatory responsibilities and instructions.
  • Creates and update transcription templates as necessary taking into account provider and department preferences.
  • Attends trainings on diverse subjects such as information technology, legal, HIPAA and regulatory compliance, billing and coding. 
  •  Other duties as assigned/directed by physician or Clinic Manager.
  • Must support and model Seven Service Care Standards.
  • Must be able to complete the physical, sensory and mental requirements of the position.

Qualifications:

  1. Wisconsin LPN license or Medical Assistant certification required. 
  2. Typing skills of 45 or greater wpm required.
  3. Education in medical terminology required. Anatomy and physiology education a plus.
  4. At least two years Medical Scribe experience preferred.
  5. Good spelling, punctuation, grammar, and oral communication skills.
  6. Proficient in Microsoft Word, experience with EPIC preferred.  Previous medical record software and transcription software preferred.
  7. Must maintain accuracy, good spelling, good grammar and a strong attention to detail.
  8. Respect for confidentiality, and dependability required.  Position requires close attention to details.
  9. BCLS Certified required.
