TITLE:                        OCCUPATIONAL THERAPIST


Department:                          Rehabilitation Services

Reports to:                Director of Rehabilitation Services

Supervisors:              None

Supervises:               Students, Clinical Work of Certified Occupational Therapy Assistants


Job Summary:

The practice of occupational therapy shall include assessment, treatment, planning, implementation, education, communication and demonstration of leadership to address the needs of patient and their families, as well as to foster a positive working relationship among professionals and ancillary staff.  This person may be caring for infants, children, adolescents, adults, and/or geriatric patients and will have the knowledge and skills to care for the physical and developmental needs of these populations.

Essential Functions:

  1. Provides direct patient care, evaluates functional needs and outcomes, consults with other specialists as needed.  Reassesses patient’s functional needs, and adjusts care plan as indicated to ensure optimal patient outcomes.
  2. Designs and implements a plan of care for the patient and significant others based on a thorough assessment.
  3. Provides age appropriate care for infants, children, adolescents, adults, and/or geriatric patients and demonstrates the knowledge and skills to care for the physical and developmental needs of these populations.
  4. Conducts therapy practice within defined standards of care.
  5. Directs the practice of support staff in order to achieve positive patient outcomes.
  6. Communicates effectively with other professional and support staff in order to achieve positive patient outcomes.
  7. Promotes and contributes to a positive, problem-solving or service recovery environment.
  8. Maintains established hospital and departmental policies and procedures, objectives, customer service guidelines, performance improvement program, safety, environmental, and infection control standards.
  9. Documents evaluations, treatment goals, and plans within 24 hours of completion.  Regularly updates documentation and maintains all required prescriptions and signatures as required by state, federal, and payer guidelines.  Completes timely discharge summaries within one month of their last appointment.
  10. Enhances professional growth and development of self and others through participation in educational programs, current literature, in-service meetings, and workshops.
  11. Provides direction and leadership to staff, students and new graduates.  Acts as a resource to promote occupational therapy within the community.
  12. Must be able to complete the physical, sensory and mental requirements of the position.

Additional Responsibilities:

  • Additional duties as may be assigned by your director/supervisor.
  • May be required to work at offsite locations.  Current clinic settings include, but are not limited too:  inpatient, Mobility Center, Crivitz, and Stephenson. 

Qualifications:

  • Current licensure in the State of Wisconsin
  •  Michigan licensure preferred. 
  • Current CPR certification and AED preferred.
  • Basic computer knowledge and typing skills required
  • Strong written and verbal skills
  • Ability to read and communicate in English
  • Additional languages preferred

Potential exposures to latex products, blood borne pathogens, hazardous materials and communicable diseases.
